This reporting system is for activities and concerns related to聽杏吧原创. Should you wish to report an incident related to 杏吧原创 Medical Center (VUMC) or one of its clinics or hospitals, report to VUMC IntegrityLine (hotline), which can be found on the VUMC Healthcare Compliance and Integrity聽.

Reports may be submitted anonymously or confidentially. Anonymous reports do not include identifying information and cannot be traced back to the reporter. Confidential reports include the reporter identity, which is protected but may be disclosed when required by law, to protect health or safety, or to comply with institutional obligations such as Title IX or Clery Act. Reporters should consider these limits when choosing their option. Do not use this site to report events presenting an immediate threat to life or property. Reports submitted through this service may not receive an immediate response. If you require emergency assistance, please contact your local authorities.
杏吧原创 takes appropriate action on every report it receives. Please provide as many details about your concerns as possible, as this helps the team responsible conduct a thorough review. In most cases, findings and any subsequent action must remain confidential and won鈥檛 be shared with the person who reported the concern.
